To resolve the selected case of Nurse Adams and Mr. Lempke, there are a few steps that I would take before administering the bolus dose of morphine. First and foremost I would politely call the attending physician and explain my hesitation about giving a dose that could cause respiratory arrest. If the Doctor remained committed to his first decision about Mr. Lempkeās pain control I would then elect to speak with Mr. Lempke. My purpose in speaking with him would be to gain a common understanding of what his priorities are as a terminally ill patient (i.e. pain relief). I would explain to him the possible consequences that may arise if given the bolus dose of morphine, and from there allow him to decide what route he would like to take. If Mr. Lempke decides that he would rather have the bolus, then I will respect his decisions and administer the pain medication (I do understand that trying to educate and inform a patient at a time when they are in an extreme amount of pain is......
